This allows me to review your info ahead of time to provide you with best service for your needs. Once I receive his form I will confirm your appointment. If you do not receive a confirmation from me then appointment was not scheduled so try again or reach out to me by text or call. This form emails directly to me and only needs to be updated if your health situation changes. Treatments are by appointment only no walk - ins . And payment by credit card or cash. Sorry I do not except insurance at this time but do except HSA just check with your provider to see if they allow it first.
Please do not arrive more than 15 minutes before your scheduled appointment. This allows for setting up and sanitizing between clients and keeping with social distancing. But please arrive at least 10 minutes before your scheduled appointment so massage can start and end on time. Late arrivals will still end on schedule. Thank you for your understanding.
